What it is, How it works

A sample measuring 1.5 inches and consisting of approximately 200 hair strands (akin to the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal quantity for screening and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, a specimen weighing 150mg is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not accessible, an equivalent amount of body hair can be gathered. When referring to head hair, it implies only the scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

There are four foundational procedures involved in processing a drug test result in a lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial handling of a sample into the laboratory’s database. This step involves verifying that the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and inputting any additional information absent from an electronic chain of custody system.

Screening is a preliminary brief analysis for substances of abuse. Although Screening is cost-effective for negating drug use in most samples, a positive screen requires confirmation for admissibility in legal proceedings. Samples that test presumptively positive in Screening necessitate further confirmation.

If a sample is presumptively positive during Screening, additional hair is taken from the original specimen and processed for Extraction. This stage involves extracting drugs from hair at considerably lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening poses significant methodological challenges.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive undergo washing prior to confirmation as necessary. The entire procedure, from Accessioning to Confirmation, is vetted according to both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.
