When taken close to the scalp, hair offers roughly a 3-month detection duration for alcohol and drugs. Hair collection is straightforward, somewhat tough to tamper with, and easy to send.

A 1.5-inch section of approximately 200 hair strands (around the thickness of a #2 pencil) near the scalp yields 100mg of hair, which is the perfect quantity for analysis and verification. For EtG enhancements and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is unavailable, an equivalent quantity of body hair can be gathered. When mentioning head hair, we refer specifically to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary stages in processing a drug test laboratory result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial sample processing phase into a lab’s system. This step includes verifying that the sample was properly sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and performing any additional data entry that an electronic chain of custody system does not cover.

Screening is an initial rapid test for drugs of abuse. Although Screening is a cost-efficient method to exclude drug use in the majority of cases, a positive screen necessitates confirmation for legal admissibility. Any samples that show a presumptive positive result in Screening require further confirmation.

If a sample shows a presumptive positive result during Screening, more hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at notably lower concentrations than other methods (e.g., urine or oral fluid), highlighting why hair drug screening is the most rigorous method.

Confirmation of any positive result from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory workflow from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
