What it is, How it works

Hair analysis is increasingly recognized as an effective method for identifying drug and alcohol consumption. By capturing biomarkers within the fibers of a growing hair strand, hair offers a long-term overview of substance use. When procured near the scalp, hair can offer a detection timeframe of up to roughly 3 months for alcohol and other substances. Hair collection is straightforward, moderately resistant to tampering, and easily transported.

A 1.5-inch segment comprising approximately 200 hairs (comparable to the thickness of a #2 pencil) near the scalp yields 100mg, ideal for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of hair is advisable. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't accessible, a similar quantity of body hair can be obtained. When referencing head hair, it pertains strictly to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing drug test results includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial incorporation of a sample into a lab’s system. This step ensures the sample was sealed and dispatched correctly, assigns a random LAN (Laboratory Accessioning Number), and addresses any supplemental data entry missing from an electronic chain of custody system.

Screening entails an initial rapid assessment for drug substances. While cost-effective for eliminating drug usage in most samples, a positive screening must be verified to hold up in court. Samples showing presumptive positives in Screening demand further confirmation.

For samples with presumptive positives at the Screening stage, additional hair is extracted from the initial sample for the Extraction phase. Here, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), underscoring hair drug screening as the most challenging method to conduct.

Confirmation of any positive screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The entire laboratory process, from Accessioning to Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
