What it is, How it works

Hair captures a prolonged history of alcohol and drug presence by embedding biomarkers within the fibers of the emerging hair strand. If taken close to the scalp, hair can offer roughly up to a 3-month detection period for alcohol and drugs. Hair is straightforward to gather, a bit difficult to tamper with, and convenient to transport.

A 1.5-inch portion of around 200 hair strands (about the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal quantity for screening and verification. For EtG, additional assays, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest measuring the sample on a jeweler’s scale. If scalp hair isn’t available, a similar amount of body hair may be obtained. When we mention head hair, it specifically refers to scalp hair. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This process is about confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any other data entry that isn’t covered by an electronic chain of custody system.

Screening involves a preliminary rapid check for substance abuse. Although Screening serves as a cost-effective means to rule out drug usage in the majority of cases, a positive finding must be verified to stand in court. Any samples that yield a presumptive positive in Screening necessitate a secondary verification.

Should a sample return a presumptive positive outcome during the Screening phase, additional hair is extracted from the initial sample for Preparation. This stage involves extracting drugs from hair at much lower concentrations than in alternative methodologies (e.g., urine or oral fluid), which is why hair drug evaluation is among the most challenging techniques to undertake.

Confirming any positive result from screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with a presumptive positive outcome are cleansed before confirmation as required. The complete laboratory sequence from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
