For tests including EtG or extending over 10 panels, a recommendation of 150mg of the sample is suggested. It is advisable to measure the sample with a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be taken. When talking about head hair, it specifically means scalp hair, while body hair pertains to other types (facial, axillary, etc.).

Process Overview

The drug test laboratory process consists of four major stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of entering a sample into the laboratory's system, ensuring it’s properly sealed and shipped, assigning a LAN (Laboratory Accessioning Number) arbitrarily, and completing any extra data input not covered by an electronic chain of custody system.

Screening provides an initial broad assessment for any drugs of abuse. Although Screening offers a cost-effective way to exclude drug use in most samples, a positive result at this stage needs confirmation for legal admissibility. Any samples with a presumptive positive result in Screening need subsequent confirmation.

If a sample shows a presumptive positive in Screening, additional hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are derived from hair at much lower concentrations compared to other testing methods (e.g., urine or oral fluid), which makes hair drug testing one of the most challenging methodologies.

Confirmation of any positive result from Screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Any presumptive positive samples undergo washing before confirmation as required. The entire processing from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair standards and is accredited to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
