If scalp hair is not available, a comparable quantity of body hair can be collected. Mention of head hair specifically refers to scalp hair only. Body hair refers to all other hair types (e.g., facial, axillary, etc.).

Process Overview

The primary steps in the laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a specimen into a laboratory’s system. This process involves confirming that the sample was sealed and sent correctly, issuing a random LAN (Laboratory Accessioning Number), and completing any further data entry not provided through an electronic chain of custody system.

Screening entails an initial rapid assessment for drug presence. Although Screening is an economical approach to exclude drug use in most samples, a positive screen needs confirmation for legal admissibility. Any samples that show a tentative positive result during Screening require additional confirmation.

If a sample shows a presumptive positive result during the Screening phase, more hair is drawn from the initial sample for Extraction. In this stage, drug substances are extracted from hair at a significantly lower concentration than in other testing methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology to perform.

For any positive result in the Screening, Confirmation is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing a presumptive positive result are cleaned before Confirmation as appropriate. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation according to ISO / IEC 17025 standards.
