For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of hair is suggested. Using a jeweler’s scale for weighing the sample is advised. If head hair isn’t available, a similar quantity of body hair can be used. When we mention head hair, we specifically mean scalp hair. Body hair indicates all other types (facial, axillary, etc.).

Process Overview

The laboratory process for handling a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab’s system. This involves checking that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not accounted for by an electronic chain of custody system.

Screening constitutes an initial rapid assessment for substances of abuse. Although Screening is a cost-efficient method for eliminating drug use in the majority of samples, a positive screen must be verified to be acceptable in legal proceedings. Any samples presumptively positive during Screening require additional confirmation.

If a sample is presumed positive during the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. In Extraction, drugs are removed from the hair at significantly lower concentrations compared to other methods like urine or oral fluid, making hair drug testing the most complex method to execute.

Confirmation of any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are cleansed before confirmation as necessary. The whole lab procedure from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair designation and aligns with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
