Body hair encompasses all other types (including facial and axillary hair).

Process Overview

The laboratory processing of a drug test result primarily consists of four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial incorporation of a sample into a lab's system. This step verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry not covered by an electronic chain of custody.

Screening provides a preliminary assessment for prohibited substances. While cost-efficient and suitable for ruling out drug presence in most cases, any positive findings require further validation to meet legal standards. Samples indicating a presumptive positive in Screening must undergo additional confirmation.

A presumptive positive determination in the Screening phase necessitates preparing additional hair from the original sample for Extraction. This process involves isolating drugs from hair at much lower concentrations compared to other techniques (such as urine or saliva), highlighting hair testing's complexity.

Subsequent to a positive screening result, Confirmation is executed using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Any samples with presumptive positives are washed before confirmation if required. The entire laboratory workflow from Accessioning through Confirmation is systematically reviewed according to both the CAP's (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
