A sample measuring 1.5 inches and roughly 200 hair strands (comparable to a #2 pencil thickness) from the scalp provides 100mg of hair, suitable for screening and verification. For EtG, extra options, and/or analyses beyond 10 panels, 150mg of the sample is suggested. We advise measuring the sample using a jeweler’s scale. If scalp hair is inaccessible, a comparable quantity of body hair can be used. "Head hair" refers exclusively to scalp hair, while "body hair" encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a specimen into a lab's system, ensuring the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry absent in an electronic chain of custody system.

Screening conducts a preliminary rapid check for drug abuse. It is an economical method to exclude drug use in most specimens; however, a positive result needs confirmation for court acceptance. Any specimens with presumptive positive findings in Screening require further validation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. This stage involves drugs being extracted from hair at much reduced concentrations than other methods (such as urine or oral fluid), making hair drug analysis one of the most complex procedures.

Confirmation for any positive Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo a wash before confirmation if necessary. The entire lab process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
