If scalp hair is absent, an equivalent volume of body hair can be utilized. In this context, head hair pertains solely to scalp hair, whereas body hair includes other forms such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result encompasses four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary process where the sample is registered in the laboratory system. This phase ensures the sample’s proper sealing and transportation, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry lacking from an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs. While it economically rules out drug use in most samples, a positive screening requires further validation for legal admissibility. Those samples that test presumptively positive in Screening necessitate a subsequent confirmation test.

Should a sample test presumptively positive during the Screening step, additional hair is extracted from the initial sample for the Extraction process. This step involves retrieving drugs from hair at significantly lower concentrations than alternative methods (such as urine or oral fluid tests), reinforcing the complexity of performing hair drug screening.

Any positive results from Screening are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Confirmatory testing includes washing presumptive positive samples as needed. The entire laboratory procedure, from Accessioning to Confirmation, is evaluated under the purview of both the CAP (College of American Pathologists) Hair designation and the accreditation standards of ISO/IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
