If scalp hair cannot be collected, an equivalent amount of body hair may be used. When mentioning head hair, it specifically refers to scalp hair, whereas body hair encompasses other types of hair (face, underarm, etc.).

Process Overview

There are four primary steps in the lab examination process of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the early stage of integrating a sample into a lab’s system. This process includes ensuring the sample was properly sealed and transported, assigning it a unique LAN (Laboratory Accessioning Number), and completing any further data input not covered by an electronic chain of custody system.

Screening is the preliminary assessment for drugs of misuse. Although Screening serves as a cost-efficient method to dismiss drug consumption for most samples, a positive result in Screening necessitates confirmation to be valid in legal proceedings. Samples testing presumptively positive in Screening require a secondary validation.

If a sample results in presumptive positivity during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. At this stage, substances are removed from hair at a much lower concentration compared to other methodologies (such as urine or oral fluid), making hair drug analysis a challenging method to execute.

Confirmation of any positive Screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positives are cleansed before confirmation as appropriate. The complete process from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
