What it is, How it works

Hair analysis has emerged as an effective mechanism for identifying drug and alcohol consumption. Hair records a prolonged timeline of alcohol and drug activity by ensnaring biomarkers within the strands of growing hair. When obtained near the scalp, hair can offer up to a 3-month detection period for alcohol and drug usage. Hair is straightforward to gather, relatively hard to tamper with, and uncomplicated to transport.

A 1.5-inch portion consisting of roughly 200 hair strands (approximately matching a #2 pencil's thickness) nearest to the scalp yields 100mg, making it an optimal sample for evaluation and affirmation. For EtG, additional tests, and/or panels exceeding ten, a collection of 150mg is advised. It is recommended to measure the specimen using a jeweler's scale. Should scalp hair be inaccessible, an equivalent amount of body hair is acceptable. Head hair specifically pertains to scalp hair, while body hair extends to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary introduction of a specimen into a lab’s protocol. This procedure ensures the sample’s proper sealing and transport, assigns a random LAN (Laboratory Accessioning Number), and supplements any absent data not recorded by an electronic custody system.

Screening offers an initial rapid assessment for drug abuse. While Screening is an efficient method for eliminating drug use in most cases, a positive screen necessitates confirmation for court acceptance. Samples with a presumptive positive in Screening mandate a follow-up confirmation.

When a sample is marked presumptively positive during Screening, additional hair is collected from the preliminary sample and readied for Extraction. During this phase, drugs are separated from hair at notably reduced concentrations than with other techniques (like urine or oral fluid), rendering hair drug screening as one of the most complex methodologies to conduct.

Any confirmed positive screening outcomes undergo validation via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation when necessary. The entire laboratory workflow from Accessioning to Confirmation is scrutinized in accordance with both the CAP (College of American Pathologists) Hair criteria and the certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
