A 1.5-inch section comprising around 200 hair strands (roughly the breadth of a #2 pencil) closest to the scalp will yield 100mg, which is suitable for screening and validation. For EtG, supplementary checks, and/or tests beyond 10 panels, a 150mg specimen is suggested. It’s advisable to weigh the specimen using a jeweler’s scale. If scalp hair isn’t obtainable, a comparable amount of body hair can be gathered. Specifically, head hair refers only to scalp hair, whereas body hair includes other varieties (such as facial or axillary hair).

Process Overview

The laboratory workflow for processing a drug test result consists of Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample enters the laboratory system. This procedure ensures that the sample was properly sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data logging absent from an electronic chain of custody system.

Screening acts as an initial swift assessment for substance abuse. While this is a cost-efficient approach to dismiss drug use in most cases, a positive outcome from screening needs affirmation for it to be valid in court. Samples returning presumptively positive in Screening require secondary confirmation.

If screening suggests a sample is positive, further hair is extracted from the primary specimen for Extraction. Drugs in this phase are extracted at a much more reduced concentration compared to other methods (like urine or oral fluids), which makes hair drug screening notably challenging to perform.

Confirmation of any positive outcome from Screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive specimens are rinsed prior to confirmation as required. The complete lab procedure from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
