When gathered near the scalp, hair can reveal drug and alcohol use over a detection period of approximately three months. Collecting hair is straightforward, challenging to tamper with, and can be transported with ease.

A 1.5-inch segment consisting of roughly 200 hairs (comparable to a #2 pencil in thickness) positioned closest to the scalp supplies 100mg of hair, which is the optimal amount for analysis and verification. For EtG, supplementary tests, and/or panels exceeding 10, a 150mg sample is advisable. It is suggested to measure the sample on a jeweler’s scale. If scalp hair is not accessible, an equivalent quantity of body hair can be used. References to head hair specifically mean scalp hair. Body hair includes all other types of hair (such as facial and axillary hairs).

Process Overview

The laboratory processing of a drug test result involves four critical steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the laboratory's system. The process ensures the sample is properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry not covered by an electronic chain of custody system.

Screening provides an initial swift evaluation for substances of abuse. Although Screening is a cost-effective method to eliminate drug use on most samples, any positive outcome from the screen must undergo confirmation for legal admissibility. Samples that screen presumptively positive require an additional confirmation step.

For samples that appear positive during the Screening phase, additional hair is extracted from the original sample and readied for Extraction. In this phase, hair is analyzed to extract drugs at concentrations lower than in other methods (such as urine or oral fluid), which makes hair drug screening a challenging procedure.

Confirmation of positive screening results employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples with presumptive positives are cleaned prior to confirmation as necessary. The complete laboratory workflow, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
