A 1.5-inch hair sample consisting of approximately 200 strands (around the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is perfect for screening and confirmation tests. For EtG, additional tests, and/or analyses surpassing 10 panels, 150mg of the sample is suggested. We advise using a jeweler’s scale to weigh the sample. If scalp hair is not present, an equivalent quantity of body hair can be collected. When referring to head hair, it exclusively means scalp hair, whereas body hair encompasses all other hair types (such as facial or axillary hair).

Process Overview

The laboratory analysis of a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into a laboratory’s system. This step includes ensuring the sample is properly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and finishing any data entry not covered by an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. Although screening is an economical method for negating drug use in most samples, any positive finding must be validated to be legally admissible. Positive indications in Screening necessitate a secondary confirmation.

Should the Screening phase indicate a presumptive positive result, additional hair is extracted from the original sample and readied for Extraction. During this phase, drugs are isolated from hair at considerably lower concentrations than in alternative methodologies (such as urine or oral fluids), making hair drug testing the most complex method to execute.

Confirming any positive screening result employs GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All samples preliminarily deemed positive undergo a washing process before confirmation as required. Every step from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair framework and the accreditation to ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
