For EtG, add-ons, and/or tests over 10 panels, a quantity of 150mg is advisable. We suggest using a jeweler's scale for measuring the specimen weight. If scalp hair is not available, an equivalent amount of body hair can be used. When referring to head hair, this indicates only scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four principal stages in the laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial insertion of a sample into a lab system, comprising verifying the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not provided by an electronic chain of custody system.

Screening entails an initial rapid check for drugs of abuse. Screening serves as a cost-efficient method to exclude drug use for most samples, though a positive screen must be confirmed to be legally admissible. Any samples assumed positive in Screening require a secondary Confirmation.

If a sample is noted as presumptively positive during Screening, additional hair is extracted from the initial sample for Extraction. Here, drugs are extracted from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Confirmation of any positive screen result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to Confirmation if necessary. The laboratory process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
