A 1.5-inch portion consisting of approximately 200 hair strands (roughly the size of a #2 pencil) taken nearest to the scalp yields 100mg, which is ideal for both screening and validation tests. For EtG, supplements, and/or tests exceeding 10 panels, 150mg is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn’t available, a comparable quantity of body hair can be gathered. When referencing head hair, we mean scalp hair specifically. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The key stages in processing a drug test outcome in the lab encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the first stage of processing a sample in a lab’s system. It ensures that the sample was correctly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not captured by an electronic chain of custody system.

Screening is a preliminary assessment for substances of abuse. Though it’s a cost-efficient method for ruling out drug use in most samples, any positive screening must be verified to be court-admissible. Samples that are presumptively positive during Screening require further confirmation.

Should a sample test presumptively positive at the Screening stage, additional hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are removed from hair at significantly lower levels compared to other techniques (such as urine or oral fluid), highlighting why hair drug screening is the most challenging methodology.

Confirmation of any positive screening results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples undergo washing before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is evaluated in line with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
