For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. It is recommended to weigh the specimen on a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair may be collected. Head hair refers specifically to scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into a laboratory’s system. This step includes verifying that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not furnished by an electronic chain of custody system.

Screening involves a preliminary rapid check for drugs of abuse. While Screening serves as a cost-effective means to dismiss drug usage in most samples, a positive result necessitates confirmation to be valid in legal settings. Samples that indicate a presumptive positive in Screening must undergo a secondary confirmation.

Upon a presumptive positive in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which makes hair drug screening a particularly challenging method to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleaned before confirmation as required. The complete laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
