If scalp hair isn’t available, an equivalent quantity of body hair can be used. References to head hair pertain to scalp hair exclusively. Body hair encompasses all other hair variants (facial, axillary, etc.).

Process Overview

Accessioning involves the sample’s initial processing into a lab’s system. This step verifies the sample’s proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not handled by an electronic chain of custody system.

If Screening indicates a presumptive positive result, additional hair from the initial sample is extracted for preparation. This stage involves extracting drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging to conduct.

Confirmation of any positive screening outcomes is completed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The complete lab process from Accessioning to Confirmation is monitored under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
