For EtG, additional tests, or tests exceeding 10 panels, it is suggested to use 150mg of the sample. It's advisable to weigh the sample using a jeweler’s scale. If no scalp hair is available, an equivalent quantity of body hair can be obtained. The term head hair specifically refers to scalp hair, while body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four principal steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system, which includes ensuring the sample was correctly sealed and dispatched, assigning a random Laboratory Accessioning Number (LAN), and completing any additional data input not covered by an electronic chain of custody system.

Screening involves a preliminary, rapid assessment for drugs of misuse. While Screening minimizes costs and excludes drug usage in many samples, a confirmed positive result is necessary for legal acceptability. Samples that show a presumptive positive in Screening require subsequent confirmation.

If a presumptive positive arises in the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), explaining why hair drug screening is challenging to execute.

Any positive Screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples undergo washing before confirmation when necessary. The complete lab procedure from Accessioning to Confirmation is assessed according to CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
