What it is, How it works

Hair assessments are now recognized as a significant means for identifying substance and alcohol use. Hair encapsulates a prolonged record of alcohol and other substances by capturing biomarkers within the strands of hair as they grow. When procured near the scalp, hair offers nearly a 3-month period for identifying alcohol and drugs. Collecting hair is straightforward, comparatively resistant to tampering, and convenient to transport.

A lock of hair measuring 1.5 inches, containing around 200 strands (about the thickness of a #2 pencil) nearest to the scalp, yields 100mg of hair, the optimal quantity for both checking and confirming. For EtG, additional examinations, and/or assessments exceeding ten panels, it is advisable to have 150mg of the sample. It's recommended to measure the sample using a jeweler’s balance. If head hair is not accessible, a similar volume of body hair may be obtained. When discussing head hair, it pertains solely to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning relates to the initial incorporation of a sample into a laboratory’s framework. This process ensures the sample was appropriately sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and executes supplementary data entry not available through an electronic chain of custody system.

Screening serves as an initial rapid assessment to detect illicit drug use. Although Screening is an economical tactic for eliminating drug usage for most samples, a confirmed result is needed for admissibility in a court setting. Any presumptively positive Screening samples indeed require subsequent confirmation.

Should a sample show presumptively positive in the Screening stage, additional hair is extracted from the original specimen and prepared for Extraction. At this point, drugs are extracted at notably lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening among the most challenging methodologies to conduct.

Confirmation for any positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples with a presumptive positive need to be cleansed before confirmation when necessary. From Accessioning to Confirmation, the entire laboratory procedure is scrutinized under both the CAP (College of American Pathologists) Hair category and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection period: Hair drug assessments can identify drug use for up to 90 days, unlike urine tests, which offer a shorter detection span.
  • Resistant to deception: Cheating on a hair drug test is highly challenging, ensuring results are more reliable.
  • Gives historical insights: It demonstrates a pattern of substance use over time, not merely recent consumption.

Limitations:

  • Not for recent usage detection: It takes roughly 5-7 days for substances to become observable in hair.
  • Expense: Hair drug assessments typically cost more than other drug testing techniques.
  • Results may vary: Elements like hair color and personal differences in hair growth can influence the presence of drug residues in the hair.

Note: Termed as "hair follicle tests", the examination is conducted on the hair strand instead of the follicle beneath the scalp
