What it is, How it works

Hair testing is increasingly recognized as an effective method for detecting alcohol and drug consumption. Hair can offer an extended history of substance use by capturing biomarkers within the fiber of the hair strand as it grows. When harvested near the scalp, the hair can reveal drug and alcohol use within roughly a 3-month detection timeframe. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch segment comprising approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is an optimal quantity for screening and confirmation purposes. For additional EtG tests and/or panels exceeding 10, 150mg of the sample is suggested. We advise measuring the specimen with a jeweler’s scale. If scalp hair is inaccessible, an equivalent quantity of body hair may be used. References to head hair specifically mean scalp hair. Body hair encompasses all other forms (facial, axillary, etc.).

Process Overview

The key phases in the lab handling of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into the lab’s system. This step ensures the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and finalizes any additional data input not captured by an electronic chain of custody system.

Screening comprises an initial rapid assessment for substances of abuse. While Screening economically eliminates drug presence in most samples, a positive result necessitates confirmation to be legally valid. Samples that show an initial positive in Screening require subsequent confirmation.

Should a specimen show a presumptive positive during Screening, additional hair is extracted for the Extraction process. In this phase, substances are drawn from hair at significantly lower levels than in other techniques (such as urine or salivary tests), making hair drug screening the most challenging technique to execute.

Verification of positive Screening findings is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples undergo washing before confirmation as required. The entire laboratory process, from Accessioning to Confirmation, undergoes scrutiny under the CAP (College of American Pathologists) Hair designation along with accreditation to ISO / IEC 17025 standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
