However, urine tests remain the most accurate for recent drug use within the past 5 days.

What it is, How it works

Hair analysis has emerged as an effective mechanism for unveiling drug and alcohol consumption. Hair strands can capture biomarkers within the fibers of the growing hair, providing a long-duration record of substance use. When clipped near the scalp, hair offers a detection timeline of up to around three months for alcohol and other substances. Hair is easy to collect, relatively resistant to tampering, and straightforward to transport.

A 1.5-inch segment consisting of approximately 200 hair strands (equivalent to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for assessment and validation. For EtG, additional tests, and/or analyses exceeding 10 panels, a sample of 150mg is advised. We suggest using a jeweler’s scale for measuring the sample. If scalp hair is unavailable, an equivalent amount of body hair can be gathered. When discussing head hair, we refer solely to scalp hair. Body hair encompasses all other hair types (such as facial and axillary hair).

Process Overview

The drug testing procedure in a laboratory setting includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s framework. This phase involves ensuring that the sample was sealed and transported correctly, assigning it a unique LAN (Laboratory Accessioning Number), and completing any outstanding information that was not included by an electronic chain of custody system.

Screening is a preliminary rapid evaluation aimed at detecting abusive substances. Although Screening is a cost-efficient approach to discount drug use for most samples, a positive result in Screening requires further confirmation for it to be viable in a legal context. Samples with a presumptive positive result in Screening necessitate a subsequent confirmation process.

If a sample registers as possibly positive during Screening, additional hair is extracted from the initial batch for Extraction. In this phase, the substance is removed from the hair at a notably reduced concentration compared to other methods (e.g., urine or oral fluid), making hair substance testing the most challenging method to conduct.

Verification of any positive result from Screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that show presumptive positivity undergo washing before confirmation as needed. The comprehensive laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair standard and the ISO / IEC 17025 accreditation measures.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
