A sample measuring 1.5 inches, comprising about 200 strands (approximately the diameter of a #2 pencil) near the scalp will yield 100mg of hair, considered the optimal sample for both testing and validation. For EtG, additional tests, and/or panels exceeding 10, a sample weighing 150mg is advisable. We suggest measuring the sample using a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair is acceptable. When discussing head hair, we refer exclusively to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary steps for processing a drug test in the lab are composed of Accessioning, Screening, Extraction, and Confirmation.

Accessioning means the preliminary processing of a specimen into the lab’s tracking system. This step involves ensuring the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and performing any remaining data input missing from an electronic chain of custody system.

Screening is the initial rapid assessment for substances of abuse. While Screening serves as a cost-effective method to negate drug usage for most specimens, a positive screen needs confirmation to be legally acceptable. Any specimens with an initial positive result in Screening require an additional confirmation step.

In cases where a sample shows a presumptive positive outcome during Screening, additional hair is taken from the original sample and readied for Extraction. During Extraction, drugs are isolated from the hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug testing one of the most complex methods to conduct.

Verification of any positive Screened result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Before proceeding with confirmation, all presumptively positive samples undergo washing as necessary. The entire laboratory sequence from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair standards and the authorization conforming to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
