What it is, How it works

Hair collected close to the scalp can offer a 3-month detection period for substances like alcohol. It's easy to collect, challenging to tamper with, and convenient to transport.

A 1.5-inch length of hair, approximately 200 strands (similar to the thickness of a #2 pencil), near the scalp provides 100mg, an optimal amount for both screening and confirmation. For EtG, add-ons, or tests beyond 10 panels, 150mg is advised. It is best to weigh the sample using a jeweler’s scale. If head hair is not available, a similar quantity of body hair can be used. When we refer to head hair, it is specific to scalp hair. Body hair includes facial, axillary, and other hair types.

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where samples are incorporated into the laboratory system. This step confirms the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input not covered by an electronic chain of custody system.

Screening is a preliminary step to evaluate samples for drugs of abuse. Although Screening is an economical approach to exclude drug presence in many samples, a positive result requires confirmation to be legally acceptable. Samples that are positive in Screening proceed to secondary confirmation.

For samples positive in Screening, additional hair from the original sample is utilized for Extraction. During this phase, drugs are extracted at a lower concentration than other methods (e.g., urine or saliva), highlighting the complexity of hair drug analysis.

Confirmation follows any affirmative screening result using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumed positive samples undergo washing before confirmation if necessary. The entire testing process from Accessioning to Confirmation adheres to CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
