A 1.5-inch section of roughly 200 strands (comparable to the girth of a #2 pencil) closest to the scalp yields 100mg of hair, the preferred sample volume for testing and validation. For EtG, enhancements, and/or analyses beyond 10 panels, a 150mg quantity is advisable. We advocate measuring the specimen with a jeweler’s scale. If scalp hair is inaccessible, a comparable quantity of body hair may be taken. References to head hair pertain only to scalp hair, whereas body hair encompasses all other types like facial and axillary.

Process Overview

There are four pivotal stages in lab processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the entry of a sample into a laboratory’s system initially. This process includes confirming the correct sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and concluding any additional data entry omitted by an electronic chain of custody system.

Screening represents an initial, brief inspection for drugs of misuse. Though economical, Screening is essential to eliminate drug use in most samples, but a positive screen necessitates confirmation to be valid in legal settings. Any samples with presumptive positives in Screening demand a subsequent confirmation.

When a sample indicates a presumptive positive in the Screening phase, additional hair from the original sample is obtained for Extraction. Here, drugs are isolated from hair at levels considerably lower than other methods (like urine or oral fluid), part of why hair analysis is the most challenging technique to execute.

Confirmation for any positive screen is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation if needed. The complete process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair endorsement and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
