Despite this, for recent drug use detection, urine tests still prove highly effective within a 5-day frame.

What it is, How it works

Hair testing is widely recognized as a significant method for identifying substance and alcohol usage. The hair captures long-term evidence of alcohol and drugs by embedding biomarkers within the fiber of the growing hair. If obtained near the scalp, hair offers around a 3-month detection timeframe for substances and alcohol. It's straightforward to gather, challenging to tamper with, and convenient to send.

A segment of 1.5 inches consisting of roughly 200 hair strands (equivalent to a #2 pencil’s width) near the scalp provides 100mg of hair, optimal for analysis and verification. For EtG, enhancements, and/or tests exceeding 10 panels, a 150mg sample is advised. The specimen should be weighed on a jeweler’s scale. If head hair isn't available, a similar amount of body hair can be used. When discussing head hair, we mean scalp hair specifically. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The principal phases within the lab processing for a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary handling of a specimen within a lab’s framework. This covers confirming the sample was appropriately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finishing any further data documentation not given by an electronic chain of custody framework.

Screening constitutes an initial rapid assessment for drug misuse. While Screening offers a cost-efficient method to rule out drug consumption for most specimens, a favorable screen must be validated to be permissible in legal settings. Any samples that are tentatively positive during Screening need subsequent confirmation.

In case a sample is tentatively positive during the Screening process, additional hair is extracted from the original sample and arranged for Extraction. Here, substances are retrieved from hair at a much reduced concentration compared to other methods (e.g., urine or oral fluid), explaining why hair drug screening is particularly difficult to execute.

Verification of any favorable screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Every tentatively positive sample undergoes a washing procedure prior to verification as essential. The entire lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair label and the recognition of ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
