What it is, How it works

Hair testing has emerged as an effective method to detect drug and alcohol usage. Hair retains a long-term record of alcohol and other drugs by entrapping biomarkers within the fibers of the developing hair strand. When collected near the scalp, hair can provide an approximately 3-month detection timeframe for alcohol and other drugs. Hair collection is straightforward, relatively challenging to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 strands of hair (comparable in size to a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred sample size for screening and confirmation. For EtG, additional tests, and/or assessments beyond 10 panels, 150mg of the sample is suggested. We advise measuring the specimen using a jeweler’s scale. If scalp hair is unavailable, a similar volume of body hair can be collected. When we mention head hair, we mean only scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab's system. This comprises confirming that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not provided by an electronic chain of custody system.

Screening subjects a sample to an initial fast check for drugs of abuse. While Screening is a cost-efficient means to eliminate drug usage in most samples, a positive screen requires confirmation for legal admissibility. Any samples deemed presumptively positive in Screening necessitate secondary confirmation.

If a sample is categorized as presumptively positive during the Screening stage, additional hair is extracted from the initial sample and prepped for Extraction. In this stage, drugs are isolated from hair in much lower concentrations than in other methods (such as urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed prior to confirmation if needed. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair specification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
