What it is, How it works

Hair testing is increasingly recognized as an essential method for detecting drug and alcohol usage. Hair can store a long-term record of substances by locking in biomarkers within the fibers of the hair as it grows. When collected just near the scalp, hair samples can provide a detection window of about three months for alcohol and other substances. Hair collection is straightforward, challenging to tamper with, and convenient to send.

A 1.5-inch hair sample comprising roughly 200 strands (comparable in size to a standard #2 pencil) taken from near the scalp gives 100mg of hair, considered optimal for analysis and confirmation. For EtG, additional testing, and tests beyond 10 panels, 150mg of the hair sample is advisable. We suggest using a jeweler’s scale for an accurate specimen weight. If scalp hair is unavailable, collecting a comparable quantity of body hair is a viable alternative. When referring to head hair, we specifically mean scalp hair. Body hair encompasses other types, such as facial and axillary hair.

Process Overview

The laboratory stages for processing drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step involving processing the sample in a laboratory environment. It entails confirming the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not available through an electronic chain of custody system.

Screening performs a preliminary check for drugs of abuse. Although it is a budget-friendly way to exclude drug use in most cases, positive screens require confirmation to be legally acceptable. Any samples identified as presumptively positive during Screening must undergo secondary confirmation.

For samples deemed presumptively positive in the Screening phase, additional hair is extracted from the original specimen for the Extraction phase. Here, drugs are processed from hair at notably lower concentrations than other methods (such as urine or oral fluid), which is why hair drug screening is considerably challenging among methodologies.

The Confirmation phase for any positive screen result involves techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleaned before confirmation as necessary. The entire laboratory procedure from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.
