What it is, How it works

Hair analysis is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged history of alcohol and drug intake by embedding biomarkers within the hair strand as it grows. When collected near the scalp, hair can offer nearly a 3-month window for detecting alcohol and drug usage. It is straightforward to gather, challenging to tamper with, and convenient to transport.

A 1.5-inch section consisting of about 200 hair strands (comparable to the diameter of a #2 pencil) nearest to the scalp provides 100mg of hair, which is optimal for screening and validation. For EtG, additional tests, and/or panels exceeding 10, a sample weight of 150mg is advised. It is suggested to weigh the specimen on a jeweler’s scale. If head hair isn't available, the equivalent quantity of body hair might be gathered. "Head hair" specifically refers to scalp hair, while "body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

The key phases in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial entry of a sample into a lab’s system. This phase involves confirming the sample was sealed and dispatched correctly, assigning it a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody process.

If a sample yields a presumptive positive result during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. This phase involves isolating drugs from hair at a far lower concentration than in other methods (e.g., urine or oral fluid), making hair drug analysis the most challenging method to execute.

Confirming any positive Screening outcomes is performed using GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positive results are cleansed if needed before confirmation. From the Accessioning to Confirmation, the entire lab procedure adheres to the CAP (College of American Pathologists) Hair designation and follows accreditation standards of ISO / IEC 17025.
