A sample of 1.5 inches consisting of approximately 200 hair strands (about the thickness of a #2 pencil) closest to the scalp delivers 100mg of hair, the optimal amount for both screening and confirmation tests. For EtG, additional tests, or over 10-panel screenings, 150mg of sample is recommended. It is advisable to weigh the sample with a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be used. For head hair, specifically, scalp hair is considered. Body hair includes all other hair types (such as facial or axillary hair).

Process Overview

The laboratory processing for a drug test result comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of processing a sample into the lab’s system. This phase ensures the sample’s proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry absent in an electronic chain of custody.

Screening is a preliminary analysis to test for substances of abuse. While cost-effective for ruling out drug use in most samples, a confirmed positive screen is required to be legally admissible. Any samples with a presumptive positive result from Screening need further confirmation.

When Screening suggests a sample is positive, additional hair from the same source is prepared for Extraction. This stage extracts drugs from hair at lower concentrations than methods like urine or oral fluid tests, showcasing the complexity of hair drug screening.

Confirmation of any presumptive positive result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all samples deemed positive undergo washing if needed. The entire Accessioning through Confirmation process is audited under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
