For EtG, additional tests, and/or examinations exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample with a jeweler’s scale. If no scalp hair is present, a similar portion of body hair can be gathered. By head hair, we mean only scalp hair, while body hair includes all other forms (facial, axillary, etc.).

Process Overview

The laboratory drug test processing involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is incorporated into a lab's system. This encompasses ensuring the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any other data entry absent from an electronic chain of custody.

In the Screening phase, a rapid initial examination for illicit substances occurs. While Screening is a cost-efficient method to eliminate drug use in the bulk of samples, a positive result must be verified for court validity. Samples with presumptive positives from Screening necessitate secondary verification.

When a sample indicates a presumptive positive during Screening, additional hair from the initial sample undergoes preparation for Extraction. In this phase, substances are drawn from hair at a significantly reduced concentration compared to other techniques (e.g., urine or oral fluid), making hair drug screening the most intricate methodology.

Any positive screening outcomes are validated using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed ahead of confirmation as required. The entire lab process, from Accessioning to Confirmation, undergoes review under CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
