A 1.5-inch hair segment consisting of around 200 strands (roughly akin to the thickness of a #2 pencil) near the scalp will yield 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, 150mg of hair specimen is advisable. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, a similar volume of body hair can be retrieved. Note that when we refer to head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the laboratory’s system. This phase includes verifying the sample's integrity in terms of sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and carrying out any additional data entry absent from an electronic chain of custody mechanism.

Screening constitutes an introductory rapid assessment for illicit substances. Despite being cost-effective for excluding drug use in most samples, a positive screen must face confirmation to be legally accepted. All samples that appear positive during Screening necessitate further confirmation testing.

In instances where Screening yields a presumptive positive outcome, additional hair is taken from the original sample for the Extraction stage. In this phase, drugs are isolated from hair at concentrations considerably lower compared to other methods (such as urine or oral fluid), which explains the complexity of hair drug screening.

Confirmation for any positive Screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples that tested presumptively positive are cleansed if required. The lab process from Accessioning to Confirmation undergoes thorough review in alignment with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
