If scalp hair is inaccessible, a similar quantity of body hair might be used. 'Head hair' refers strictly to scalp hair, while 'body hair' encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four primary steps in the laboratory processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is processed into a lab's system. This step ensures proper sealing and shipment of the sample, assigns a unique LAN (Laboratory Accessioning Number), and includes additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary evaluation to detect drugs of abuse. While it's a cost-efficient method to dismiss drug use in the majority of samples, any positive result necessitates confirmation for legal acceptability. Samples with a presumptive positive in Screening require further confirmation.

For samples deemed presumptively positive in Screening, more hair is extracted from the primary specimen and prepared for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than other methodologies (such as urine or oral fluid), making it the most challenging method to carry out.

Positive screening confirmations are verified using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if required. The overall laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.
