For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair is not present, an equivalent quantity of body hair can be gathered. When discussing head hair, it specifically means scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into a laboratory’s system. This step involves ensuring the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry beyond what an electronic chain of custody system provides.

Screening is an initial rapid assessment for drugs of abuse. While it is a cost-effective method to rule out drug use in most samples, a positive result needs confirmation for court admissibility. Samples that test presumptively positive in Screening do necessitate a secondary confirmation.

If a sample tests positive at the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. During this phase, drugs are extracted from hair at a considerably lower concentration than in other techniques (e.g., urine or oral fluid), which is why hair drug screening is one of the most challenging methodologies to execute.

Confirmation for any positive screening outcome is done via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing before confirmation as required. The complete laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
