A 1.5-inch segment of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal quantity for both screening and confirmation. For tests like EtG, add-ons, and/or panel tests above 10, it's advisable to collect 150mg of hair. It is suggested to weigh the collected hair using a jeweler’s scale. In cases where scalp hair isn't available, a comparable quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair; body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The process of laboratory drug testing involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is first processed into the lab’s system. This step ensures the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry that isn't part of an electronic chain of custody system.

Screening conducts an initial, swift evaluation for commonly abused substances. Although Screening is an economical approach to exclude drug use in most samples, any positive screening result requires confirmation to hold up in legal proceedings. Samples that test presumptively positive in Screening necessitate further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original specimen and set up for Extraction. This phase involves extracting drugs from hair at much lower concentrations than other methods (such as urine or oral fluid), which is why hair drug testing is one of the most challenging procedures.

Confirmation of any positive screening result is performed using techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing as required before confirmation. The complete laboratory process, from Accessioning to Confirmation, complies with both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
