What it is, How it works

Hair testing is now recognized as a potent method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and drug usage by encapsulating biomarkers within the fibers of the growing strand. When collected near the scalp, hair can offer a detection window of up to approximately 3 months for alcohol and drugs. Hair collection is straightforward, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch sample consisting of roughly 200 hair strands (about the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, a specimen of 150mg is advised. We suggest using a jeweler’s scale for specimen weighing. If scalp hair is not available, a comparable quantity of body hair can be collected. References to head hair pertain solely to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The primary stages in laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a lab’s system. This involves checking that the sample was sealed and shipped correctly, generating a random LAN (Laboratory Accessioning Number), and finishing any additional data entry not supplied by an electronic chain of custody system.

Screening offers an initial swift examination for drug abuse. While Screening is a cost-efficient method to eliminate drug usage in most samples, a positive screen needs confirmation to be permissible in court. Presumptive positives in Screening require a secondary confirmation.

For presumptive positives in the Screening phase, more hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are removed from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which renders hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The whole laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
