What it is, How it works

Hair captures a comprehensive timeline of alcohol and drug usage by securing biomarkers within the strands as they grow. If collected near the head, hair offers a detection timeframe of up to 3 months for identifying alcohol and drugs. Collecting hair is straightforward, hard to tamper with, and straightforward to transport.

A sample approximately 1.5 inches long, consisting of about 200 hair strands (similar in size to a #2 pencil) nearest to the head, will yield 100mg of hair, which is the perfect amount for testing and validation. For EtG, additional elements, and tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale for weighing the specimen is suggested. Should scalp hair be inaccessible, an equivalent volume of body hair can be used. When mentioning head hair, we refer only to scalp hair. In contrast, body hair includes all other types (facial, underarm, etc.).

Process Overview

The key steps involved in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial input of a sample into a lab system. This encompasses ensuring that the sample was properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any remaining data input not provided by an electronic documentation system.

Screening involves a preliminary assessment for substance abuse. Though cost-efficient for excluding drug usage in most samples, a positive outcome in Screening necessitates verification to be legally valid. Any samples showing potential positive results during Screening require additional confirmation.

Should a sample show a potential positive result during Screening, a larger hair portion is extracted from the original sample and readied for Extraction. In Extraction, drugs are removed from hair at considerably lower levels compared to other methods (e.g., urine or saliva), which explains why hair drug analysis is one of the most challenging methods to execute.

Confirmation of any positive outcome in Screening is performed using GC/MS, GC/MS/MS or LC/MS/MS. Before confirmation, all likely positive samples are cleansed if required. The entire processing from Accessioning to Confirmation is examined per both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
