For comprehensive testing, including EtG and panels exceeding 10, a 150mg hair specimen is advisable. It is best to measure this amount using a jeweler’s scale. If scalp hair is inaccessible, body hair of equivalent mass can be used. Specifically, 'head hair' refers solely to scalp hair, while 'body hair' encompasses all other types such as facial and axillary hair.

Process Overview

The laboratory workflow for processing drug test samples involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory's database, ensuring the sample's seal and shipment were handled correctly, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data not covered by an electronic chain of custody system.

Screening is a preliminary assessment to identify potential drugs in most samples efficiently. Despite its cost-effectiveness, a positive result from Screening needs confirmation for legal admissibility. Any sample with a preliminary positive in Screening requires further confirmation testing.

For samples with presumptive positive findings in Screening, additional hair is extracted from the original specimen for the Extraction phase. This phase involves isolating drugs from hair at significantly lower concentrations compared to other methods (such as urine or saliva samples), making hair testing one of the most challenging methods to execute.

Every presumptive positive result undergoes Confirmation testing using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before confirmation, samples identified as presumptively positive are cleaned as necessary. The complete laboratory process from Accessioning through Confirmation is audited in line with both the CAP (College of American Pathologists) Hair protocols and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
