If scalp hair is not accessible, an equivalent quantity of body hair can be used. Regarding head hair, this specifically pertains to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary phases in the laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the initial step of incorporating a sample into a laboratory system. This step includes validating the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and finishing additional data entry if not covered by an electronic chain of custody system.

Screening consists of a preliminary quick check for substances. While Screening is an economical approach to eliminate drug use for most samples, a positive result necessitates confirmation to be legally viable. Any samples indicating a presumptive positive result during Screening require further confirmation.

If a sample is deemed presumptively positive in the Screening phase, more hair is extracted from the initial sample and arranged for Extraction. In this phase, drugs are removed from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), validating why hair drug testing is the most complex methodology to execute.

Confirmation of any positive Screening outcome is achieved using GC/MS, GC/MS/MS or LC/MS/MS techniques. All presumptive positive samples go through a wash process as necessary before confirmation. The whole laboratory procedure from Accessioning to Confirmation is supervised under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
