What it is, How it works

Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of the hair as it grows. When cut close to the scalp, it provides up to a three-month detection window for drugs and alcohol. Hair collection is straightforward, challenging to tamper with, and easy to transport.

A 1.5-inch section of around 200 hair strands (equivalent to a #2 pencil in size) near the scalp yields 100mg of hair, an optimal amount for screening and verification. For EtG, additional tests, and/or panels exceeding 10, 150mg is suggested. A jeweler’s scale is advised to weigh the sample. If scalp hair is unavailable, a similar quantity of body hair may be used. "Head hair" refers specifically to scalp hair, while "body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

Lab processing of drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample’s initial integration into a lab’s system, which includes verification of proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplemental data entry not covered by an electronic chain of custody system.

Screening is a preliminary check for drug presence. While it is a cost-efficient method for excluding drug use in most cases, any positive finding needs confirmation before it is court-admissible. Samples that are preliminarily positive during Screening require a subsequent verification.

Should a sample test presumptive positive during the Screening phase, additional hair is taken from the original sample for Extraction. Drugs are extracted from hair at notably lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening particularly challenging.

Verification of any positive screen result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to verification, all presumptive positive samples are cleansed as necessary. The complete laboratory procedure from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
