However, for drug use within a 5-day period, urine tests remain the most precise option.

What it is, How it works

The analysis of hair has become recognized as an effective method for detecting drug and alcohol usage. Hair contains a long-term record of alcohol and other substances because it captures biomarkers within the fibers of the growing hair strand. When taken near the scalp, hair can present up to roughly a 3-month window for detecting alcohol and other substances. Hair is easy to collect, relatively hard to tamper with, and convenient to transport.

A sample of 1.5 inches comprising around 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, the ideal specimen for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is advised. We suggest measuring the specimen with a jeweler’s scale. Should scalp hair not be accessible, an equivalent quantity of body hair can be collected. When mentioning head hair, it refers strictly to scalp hair. Body hair includes other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test encompasses four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory’s system. This step checks that the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and fills in any additional data entry absent from an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Though Screening is a cost-efficient method to rule out drug usage for most samples, a positive screen must be validated to be acceptable in court. Any samples that are presumptively positive in Screening indeed require secondary confirmation.

If a sample turns out presumptively positive in the Screening phase, additional hair is extracted from the original specimen for Extraction. During this phase, drugs are extracted from hair at a lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is regarded as the most challenging methodology to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positive are cleaned before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
