A 1.5-inch segment of roughly 200 hair strands, equivalent to a #2 pencil, ideally provides 100mg of hair for both screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, a 150mg sample is advisable. Using a jeweler’s scale for weighing the specimen is recommended. In cases where scalp hair is not available, an equivalent amount of body hair can be collected. "Head hair" refers specifically to scalp hair, whereas "body hair" includes all other types, like facial and axillary hair.

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into the lab's system. This step ensures the sample's proper sealing and shipping, assigns a unique Laboratory Accessioning Number (LAN), and includes any extra data entry that an electronic chain of custody system doesn't cover.

Screening is a preliminary assessment for drugs of abuse. While Screening is a budget-friendly method to preclude drug usage in most samples, court-admissible results require a positive screen to be confirmed. Any samples that test presumptively positive must undergo a secondary confirmation.

If a sample shows a presumptive positive during Screening, additional hair is taken from the specimen and readied for Extraction. This stage involves extracting drugs from hair at much lower concentrations than other methods like urine or oral fluid testing, which makes hair drug screening particularly challenging.

The Confirmation stage uses GC/MS, GC/MS/MS, or LC/MS/MS to verify positive screening results. Presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, adheres to both the College of American Pathologists (CAP) Hair standards and ISO / IEC 17025 accreditation.

The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Biomarkers and Pharmacokinetics: The identification and validation of reliable biomarkers and pharmacokinetic studies are fundamental aspects of drug testing and analysis. The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
