What it is, How it works

Hair testing is now regarded as an effective method for identifying drug and alcohol consumption. Hair provides a prolonged record of alcohol and drugs by embedding biomarkers in the hair strand's fibers as it grows. When taken near the scalp, hair can reveal a detection window for up to about 3 months for alcohol and drugs. Hair collection is straightforward, fairly hard to tamper with, and convenient to transport.

A 1.5-inch segment of approximately 200 hair strands (roughly the dimension of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal amount for evaluation and validation. For EtG, add-ons, and/or panels exceeding 10, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. Should scalp hair be absent, an equivalent quantity of body hair may be gathered. When mentioning head hair, it pertains exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample into a lab's system. This step verifies that the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and finalizes any additional data entry absent from an electronic chain of custody system.

Screening entails an initial swift check for drug abuse. Screening serves as a cost-efficient measure to negate drug usage in most samples, but a positive screening requires confirmation to be court-admissible. Samples that are preliminarily positive in Screening necessitate subsequent confirmation.

Should a sample be preliminarily positive during the Screening phase, additional hair is retrieved from the initial sample and prepped for Extraction. In this process, drugs are extracted from hair at concentrations lower than those found in other methods (such as urine or oral fluid), highlighting hair drug screening as a challenging method to execute.

Confirmation of any positive screening is undertaken via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation, as required. The complete lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
