A 1.5-inch section consisting of roughly 200 hair strands (about the size of a #2 pencil) near the scalp yields 100mg, the perfect amount for both screening and verification. For EtG, additional tests, and panels over 10, 150mg of sample is advised. We suggest using a jeweler's scale for weighing. If head hair isn't available, body hair in an equivalent amount can be used. When mentioning head hair, we refer solely to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The main steps in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial entry into a laboratory's system. This step verifies that the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any additional information entry absent from an electronic chain of custody system.

Screening is a preliminary check for substance use. Although it's a cost-effective means to rule out drug use in most samples, positive screens require confirmation to be valid in legal settings. Any presumptive positives found in screening must undergo secondary confirmation.

Presumptive positives in the Screening phase lead to further sample preparation for Extraction. At this stage, drug substances are extracted from hair at lower concentrations compared to urine or oral fluid, which makes hair drug testing the most challenging methodology to execute.

GC/MS, GC/MS/MS, or LC/MS/MS methods confirm any positive results. All samples labeled presumptive positive undergo necessary washing before confirmation. The complete lab process from Accessioning to Confirmation is examined under both the CAP designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
