What it is, How it works

Hair testing is increasingly recognized as a potent means for identifying drug and alcohol use. Hair records a long-term account of alcohol and drug intake by embedding biomarkers within the fibers of the developing hair strand. When extracted near the scalp, hair can provide up to a three-month detection window for alcohol and drugs. Hair collection is straightforward, relatively hard to tamper with, and easy to transport.

A 1.5-inch hair sample consisting of about 200 strands (comparable to the girth of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal quantity for analysis and verification. For EtG, additional panels, and/or tests exceeding 10 panels, it is advised to use 150mg of the sample. Utilizing a jeweler's scale for weighing the sample is recommended. If scalp hair isn't available, an equivalent amount of body hair may be used. When mentioning head hair, we exclusively refer to scalp hair. Body hair comprises all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the primary processing of a sample into a lab’s system. This process includes checking that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry not given by an electronic chain of custody system.

Screening involves an initial swift examination for drug abuse. Although Screening offers a cost-effective way to dismiss drug usage in most samples, a positive screen must be confirmed to be valid in legal procedures. Any samples that are presumptively positive in Screening require a secondary confirmation.

If a sample turns out presumptively positive in the Screening stage, more hair is extracted from the initial specimen and prepped for Extraction. In this phase, drugs are separated from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Any positive screening result confirmation is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples undergo washing before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

  • Incapable of detecting recent uptake: Drugs become detectable in hair after approximately 5-7 days.
  • Expense: Hair drug tests typically cost more than other drug testing approaches.
  • Result variability: Elements such as hair color and individual differences in hair growth can impact the concentration of drug metabolites in the hair.

Note: Despite frequently being labeled as "hair follicle tests", the examination actually analyzes the hair strand, not the follicle beneath the scalp.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
