What it is, How it works

When collected near the scalp, hair can present an approximate 90-day detection period for alcohol and drugs. Hair is straightforward to gather, relatively hard to tamper with, and convenient to transport.

An ideal sample for screening and validation is a 1.5-inch section of around 200 hair strands (comparable to the size of a #2 pencil) taken closest to the scalp, yielding 100mg of hair. For EtG, add-ons, and/or tests surpassing 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not available, an equivalent quantity of body hair can be gathered. When we mention head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a lab's system. This involves verifying the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

The Screening step involves a preliminary check for substances of abuse. While it’s a cost-efficient way to dismiss drug usage in most samples, a positive screen needs confirmation to be court-acceptable. Any samples preliminarily positive in Screening necessitate additional confirmation.

If a sample preliminarily tests positive in Screening, additional hair is taken from the initial sample and ready for Extraction. During this phase, substances are isolated from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug testing one of the most challenging methodologies.

Any positive result from Screening undergoes Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All samples preliminarily positive are cleansed before confirmation as needed. The entire lab process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
