When cut near the scalp, hair can reveal a detection window stretching up to around three months for alcohol and other substances. Hair collection is straightforward, relatively tamper-proof, and easy to transport.

An ideal sample includes a 1.5-inch segment of approximately 200 hair strands (comparable in thickness to a #2 pencil) nearest the scalp, providing 100mg of hair for initial testing and verification. For EtG, additional testing, and/or analysis beyond 10 panels, a 150mg sample is preferred. It is advised to weigh the sample using a jeweler's scale. If scalp hair is not accessible, an equivalent quantity of body hair may be used. When discussing head hair, it specifically refers to scalp hair. Body hair encompasses all other types such as facial or axillary hair.

Process Overview

The key phases in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of entering a sample into a laboratory's system. This includes checking that the sample was sealed and delivered correctly, assigning a unique Laboratory Accessioning Number (LAN), and finalizing any supplementary data entry absent from an electronic custody system.

Screening serves as an initial rapid assessment for substance misuse. Screening provides an economical approach to exclude drug usage in the majority of samples; however, a positive result necessitates verification for legal admissibility. Samples that are tentatively positive in Screening require further confirmation.

Should a sample be presumptively positive during Screening, additional hair will be extracted from the initial specimen to prepare for Extraction. At this phase, drugs are separated from hair at far lower concentrations than those seen in urine or saliva tests, which is why hair drug screening is the most challenging methodology.

Any positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All tentative positive samples are cleansed as needed before confirmation. From Accessioning to Confirmation, the entire laboratory process adheres to both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
