What it is, How it works

Hair testing has gained recognition as an effective instrument in identifying drug and alcohol consumption. Hair provides a long-term record of alcohol and other drugs by integrating biomarkers within the fibers of the developing hair strand. When collected near the scalp, hair can offer up to roughly a 3-month detection window for alcohol and other drugs. Hair is straightforward to collect, relatively hard to tamper with, and convenient to ship.

A 1.5-inch sample comprising approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal quantity for screening and confirmation. For EtG, additional panels, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest utilizing a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent volume of body hair may be obtained. When referring to head hair, we specifically mean scalp hair. Body hair refers to all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result primarily involves Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of a sample into a laboratory's system. This entails confirming that the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and filling in any additional data not provided by an electronic chain of custody system.

Screening consists of an initial quick assessment for drugs of abuse. While Screening is an economical method to rule out drug usage in the majority of samples, a positive result requires confirmation for legal admissibility. Any samples presumed positive in Screening necessitate a secondary confirmation.

If a sample is presumed positive during the Screening stage, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a considerably lower concentration compared to other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if needed. The comprehensive laboratory process from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
