What it is, How it works

A 1.5-inch segment of roughly 200 hair strands (similar to the dimension of a #2 pencil) nearest to the scalp yields 100mg of hair, which is ideal for both screening and confirmation. For EtG testing, additional testing panels beyond 10, 150mg of the sample is advisable. It's recommended to use a jeweler’s scale for weighing the sample. If scalp hair is absent, a similar quantity of body hair may be gathered. When head hair is referenced, it indicates only scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The four fundamental stages in laboratory processing of drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample into the lab’s system. This process includes confirming that the sample was sealed and dispatched correctly, issuing a random LAN (Laboratory Accessioning Number), and fulfilling any supplementary data entry not conveyed by an electronic chain of custody system.

Screening entails an initial rapid evaluation for drugs of abuse. While Screening is an economical method to exclude drug use in most samples, a positive screen must be confirmed to be legally defensible. Any samples that appear positive in Screening require a subsequent confirmation.

When a sample tests positive in the Screening phase, additional hair is extracted from the original sample to prepare for Extraction. In this phase, drugs are removed from hair at a slightly lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is among the most challenging to conduct.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed prior to confirmation as necessary. The entire laboratory procedure, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair guidelines and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
